Share of Cost, Know how to appeal if you think the hospital is discharging you too soon.

You should appeal the hospital’s decision to discharge you if you think you are being told to leave too soon. To appeal, first consult the Important Message from Medicare notice, which the hospital should provide you at least once during your inpatient hospital stay. Among other things, the notice tells you how to request a review of your case by the Beneficiary and Family-Centered Care Quality Improvement Organization, often called the Quality Improvement Organization (QIO).

To begin the appeal, call the QIO listed on your notice by midnight of the day of your discharge. The QIO should make a decision within 24 hours. If the appeal is successful, you can remain in the hospital, and Medicare or your Medicare Advantage Plan will continue to cover your care. You are not responsible for the cost of your care during this first appeal, even if you are unsuccessful. If you are unsuccessful, there are additional levels of appeal you can use – but you may have to pay for the care you receive if those appeals are unsuccessful.

As you may already know, dental coverage is not typically coverage, in your basic medicare. Original Medicare (Part A for hospital coverage and Part B for medical coverage) generally does not include routine dental coverage, such as dental exams, cleaning’s, fillings, crowns, and bridges. There are, however, some exceptions to this rule.

You may be able to get dental coverage under some Medicare Advantage plans. Medicare Advantage plans (Medicare Part C) can include extra benefits like routine dental, routine vision, and Medicare prescription drug coverage.

Discharge planning helps ensure that you and your caregivers have the information, resources, and care you need as you transition from the hospital to your home or other living arrangement. A discharge plan should identify your post-hospital care needs and the most appropriate next setting for your care. All hospitals must follow certain guidelines when preparing hospital inpatients for discharge. Although Medicare recommends that all inpatients receive an in-depth discharge plan, hospitals must only provide discharge plans to Medicare patients who were screened and found to be at high risk of complications . Keep in mind that Medicare discharge planning requirements and recommendations apply whether you have Original Medicare or a Medicare Advantage Plan. Also, note that Medicare recommends but does not require discharge planning if you are a hospital outpatient.

Share of Cost, NO2 air pollution increases allergenicity in ragweed pollen

Together with the Research Unit Protein Science and the Institute for Environmental Medicine of Technische Universität München as well as the research consortium UNIKA-T and the Christine Kühne – Center for Allergy Research and Education, researchers of the Institute of Biochemical Plant Pathology (BIOP) studied how nitrogen oxides affect the pollen of the plant. Specifically, they fumigated the plants with various concentrations of NO2, which e.g. is generated during combustion processes of fuel. “Our data showed that the stress on the plant caused by NO2 modulated the protein composition of the pollen,” said first author Dr. Feng Zhao. “Different isoforms of the known allergen Amb a 1 were significantly elevated.” In addition, the scientists observed that the pollen from NO2 treated plants have a significantly increased binding capacity to specific IgE antibodies* of individuals allergic to Ambrosia. This is often the beginning of an allergic reaction in humans.

Share of Cost, How to treat bee and wasp stings

Most bee stings can be treated without medical attention. First aid for someone who has been stung by a bee or wasp includes a number of dos and don’ts.  bee sting

DO:

* Stay with the person to watch out for any severe reaction that could develop

* Call for urgent medical help if there are signs of a severe allergic reaction

* Remove the stinger promptly if it remains – honey bee stingers are barbed and usually remain in the skin, and prompt removal is needed since the injection mechanism continues

* To remove the stinger, wipe over it with a piece of gauze, or scrape a finger nail, piece of card or a bank card over it

* Remain calm – walk away calmly since wasps and hornets can sting again (they do not usually leave a stinger)

* Wash the site of the sting with plain soap and water

* Apply a cold compress – ice, frozen peas or cold cloth to reduce swelling

* Offer aspirin or acetaminophen if desired to reduce pain; sprays or creams containing anesthetic and antihistamine are available from pharmacists, as are oral antihistamines for reducing swelling.

DON’T:

* Leave the person alone – they may develop a severe reaction

* Use tweezers to remove the stinger

* Squeeze the stinger in an attempt to remove it – this can cause more venom to be injected

* Scratch the sting – this could aggravate the problem and lead to an infection

* Panic, Waving around will not help, and wasps and hornets do not usually leave a stinger so they may sting again

* Use calamine lotion, vinegar or bicarbonate of soda – these are not recommended treatments, and the aim of neutralizing the acidic venom with the latter two is pointless since the venom gets deep into the tissues

* Burst any blisters that develop since this can lead to infection.

In the journal Laboratory Investigation, a team from the University of Texas Medical Branch (UTMB) at Galveston describes how the experimental drug TP508 showed promising results in tests on mice.

Exposure to nuclear radiation in the event of an incident has the potential to injure and kill thousands of people. Emergency measures that can be given in the first 24 hours to limit the damage to the body until better medical care is available can save lives, say researchers who have found a drug that appears to fit this need.

They note how a single injection of TP508, a regenerative peptide, significantly increased survival in mice after exposure to nuclear radiation, even though it was given 24 hours after the exposure occurred.

The discovery explains how two particular proteins, Tollip and Tom1, work together to contribute to the turnover of cell-surface receptor proteins that trigger inflammation.
